A health care proxy is a person who has been given authority to make health and medical care decisions for another person in the event of their incapacity to make informed choices.


The health care proxy can only be designated by a legal document that goes into effect when the person has been certified by a doctor as incapable of making decisions.


Anyone over the age of 18 can be named as a health care proxy, as long as they are of sound mind and judgement.
